Ensuring a Smooth Transition to NDIS Long-Term Accommodation: Tips and Guidance

Transitioning to long-term accommodation under the National Disability Insurance Scheme (NDIS) can seem daunting. However, with the right preparation, guidance, and an understanding of NDIS support arrangements, it can be a seamless process.

  1. Understanding the Transition Period: From April 19th, 2022, NDIS has implemented measures to support changes in support needs that impact the amount, frequency, intensity, and ratios of support delivered. This includes funding a minimum of 12 weeks at the previous plan level to support the transition to new support arrangements.
  2. Plan Ahead: Collaborate with your support coordinator or case manager to understand the changes to your supported independent living funding, time periods, and budget amount specified in your plan.
  3. Customized Transition Approach: In instances where more than 12 weeks are needed for transition, providers are expected to work with participants to develop a customized transition approach. This approach helps support a gradual adjustment to the new level of funding.
  4. Communication is Key: Regular communication with your service provider is vital. Ensure that they are aware of your new plan specifications, including the weekly value that can be claimed over the transition period, and the value to be claimed post this period.
  5. Visit Before Moving and Pack Essentials: Familiarize yourself with the new accommodation and pack personal items to create a home-like environment.
  6. Maintain Regular Routines and Stay Connected: Keep your daily routines and connections with family, friends, and community consistent during the transition.
